Photocopy as many forms as needed. Only official entry forms will be accepted. Any unofficial forms will be returned to the sender.

Please double check all entry forms to ensure correct levels, disciplines etc. No changes will be made to the final program.

Entries will be accepted until December 15th, 2017 on a first come, first serve basis. All entries must be postmarked by the closing date — entries received after this date will only be accepted if space permits.

Payment for entry fees is accepted by money order or certified cheque only. Post-dated cheques are NOT accepted. (Please see entry forms for fees).

All accepted entries received after the closing date of December 15th, 2017 will be charged the following late entry fee amount:

  • Solos: additional $25
  • Duos/Trios: additional $25/person
  • Groups: additional $10/person

Withdrawals may be made, NO REFUNDS will be given. Withdrawals due to injury will only be refunded with a doctor’s note. Refunds are issued to studio owners only, NOT to individuals.

Entry forms are available to studio owners only.

Please enclose all Entry forms and Summary of Entry form along with a money order or certified cheque only made payable to Shy-Ann Dance Productions.
